What Is a Health Home?

A Health Home is not a place—it is a team-based approach to care. Your team of service providers works closely with you and your child’s caregiver to ensure you receive the services you need and want to stay healthy. Once enrolled, your family will be assigned a Care Manager who will help develop a personalized Plan of Care that supports your child’s social, emotional, mental, and physical well-being now and in the future.

Who Is Eligible?

Children and youth ages 0–20 with active Medicaid coverage may be eligible if they have experienced physical, social, or emotional challenges that impact healthy development or their ability to achieve life goals.

Hope For Youth is a proud member of the Collaborative for Children and Families (CCF)—a Health Home made up of a large network of medical and behavioral health providers specializing in services for children and families.

Hope For Youth Care Management Services

A Care Manager is a caring, experienced professional dedicated to helping your family succeed. Your Care Manager will support you by providing:

  • Appointment Management
    Helping you keep track of appointments and ensuring your child’s needs are met.
  • Personalized Planning
    Working with your family to understand your goals and create a care plan tailored to your needs.
  • Ongoing Support
    Checking in regularly with your family and communicating your needs and concerns to service providers.
  • Access to New Services
    Connecting your child to additional programs and supports through HFY and the wider CCF network.
  • Family Support
    Offering continuous guidance, advocacy, and encouragement.
  • No Cost to Families
    Care Management services are fully covered by Medicaid for those who qualify.
